The User acknowledges possessing the independent and exclusive right to dispose of non-core aspects of their bodily rights (i.e., rights not involving life safety or grievous bodily harm). The User hereby exercises this indefeasible right of disposition and voluntarily waiv

es any right to claim compensation for minor damages against the Witness resulting from the exercise of such right.第二條：行為能力與自願性聲明Article 2: Capacity and Declaration of Voluntariness本人鄭重聲明：本人已成年，無精神疾病史，具備完全民事行為能力及充分的理解與承諾能力。

The User solemnly declares: I am of legal age, have no history of mental illness, and possess full civil capacity and the competency to understand and consent.本人對本協議所涉事項及潛在後果具有清晰、正常的理解能力。本承諾系本人在完全清醒、理智、並未受到任何形式的暴力、脅迫、欺詐或不當影響（Undue Influence）的情況下做出的真實意思表示。本承諾長期有效，除非雙方另有書面終止協議。

The User has a clear and normal understanding of the matters and potential consequences involved in this Agreement. This consent is a true expression of intent made while fully awake, rational, and free from any form of violence, duress, fraud, or undue influence. This consent shall remain valid for the long term unless terminated in writing by both parties.第三條：特定身體改造項目的確認Article 3: Confirmation of Specific Body Modifications本人知曉並同意，本人將基於自身審美偏好及個人意願，接受永久性的身體改造，具體包括：Prince Albert (PA) 穿環、紋身及佩戴鎖定裝置（Locking Device）。

The User acknowledges and agrees to undergo permanent body modifications based on personal aesthetic preference and volition, specifically including: Prince Albert (PA) piercing, t

attooing, and the wearing of a locking device.關於上述改造的具體部位、圖案設計及實施方案（詳見附錄1），雖可能參考了見證人的建議或由見證人提供參考圖樣，但最終均經本人獨立審視、確認並採納。本人確認該決定系本人的獨立選擇。

Regarding the specific locations, designs, and implementation plans (see Appendix 1), although they may reference suggestions or designs provided by the Witness, they have been independently reviewed, confirmed, and adopted by the User. The User confirms that this decision is an independent choice.第四條：第三方操作與風險自擔Article 4: Third-Party Execution and Assumption of Risk本人承諾，將自行尋找並聘請具有合法執照及專業資質的第三方人員（如紋身師、穿孔師）進行上述操作。本人明確知悉，見證人僅作為陪同者或建議者，並非實際操作人。

The User undertakes to independently seek and engage licensed and qualified third-party professionals (e.g., tattoo artists, piercers) to perform the aforementioned procedures. The User explicitly acknowledges that the Witness acts solely as a companion or advisor and is not the practitioner performing the procedures.本人充分了解紋身、穿環及佩戴鎖定裝置屬於永久性身體改造，並充分認知其中可能存在的健康風險，包括但不限於感染、過敏、排異反應、神經損傷或疤痕增生。本人承諾“自甘風險”（Assumption of Risk），自行承擔所有相關後果及醫療責任，並放棄就上述風險追究見證人任何法律責任的權利。

The User fully understands that tattooing, piercing, and locking devices constitute permanent body modifications and is fully aware of the potential health ris

ks involved, including but not limited to infection, allergies, rejection reactions, nerve damage, or scarring. The User agrees to "Assume the Risk," bearing all related consequences and medical liabilities, and waives the right to hold the Witness legally liable for any of the aforementioned risks.第五條：證據保全與隱私授權Article 5: Evidence Preservation and Privacy Authorization本協議於公開場所（Public Place）簽署，本人確認簽署過程處於公共視野內，無任何物理隔絕或隱秘性。

This Agreement is signed in a public place. The User confirms that the signing process takes place within public view, without physical isolation or secrecy.為證明本人簽署協議時的精神狀態（Capacity）及意思表示真實（Voluntariness），本人明示授權見證人對簽署過程進行全程錄音錄像。本人確認該錄製行為不構成對本人隱私權（Privacy）、肖像權（Portrait Right）及個人信息權益的侵犯。

To prove the User's mental capacity and veracity of intent (Voluntariness) at the time of signing, the User expressly authorizes the Witness to video and audio record the entire signing process. The User confirms that such recording does not constitute an infringement of the User's Privacy, Portrait Rights, or Personal Information rights.該錄音錄像資料僅限用於法律維權、證據保全及應對可能的糾紛解決。除非經本人書面同意或司法機關強制要求，見證人不得將該錄像公開、傳播、上傳網絡或用於任何商業/娛樂用途。

Such video and audio recording materials are strictly limited to

legal defense, evidence preservation, and dispute resolution. Unless with the User's written consent or required by judicial authorities, the Witness shall not disclose, disseminate, upload to the internet, or use the recording for any commercial or entertainment purposes.本人承諾，在未來任何針對見證人的法律程序中，承認該錄像作為直接證據（Direct Evidence）的效力，並放棄以“侵犯隱私”或“未經同意”為由排除該證據的抗辯權利。

The User undertakes to acknowledge the validity of the recording as Direct Evidence in any future legal proceedings against the Witness and waives the right to object to the exclusion of such evidence on the grounds of "invasion of privacy" or "lack of consent."

第六條：適用法律、管轄權與爭議解決本協議的解釋、效力及執行，以及因本協議引起或與之相關的任何爭議，均排他性地適用美國內達華州（State of Nevada）的法律，並排除其衝突法規則。承諾人明確放棄引用任何其他司法管轄區（包括但不限於中國或協議簽署地）法律來解釋或挑戰本協議條款的權利。

This Agreement, its interpretation, validity, and enforcement, shall be exclusively governed by the laws of the State of Nevada, USA, without regard to its conflict of laws principles. The User expressly waives the right to invoke the laws of any other jurisdiction (including but not limited to the PRC or the location of signing) to interpret or challenge the terms herein.雙方同意，因本協議引起或與之相關的任何民事訴訟或法律程序，必須且只能提交至美國內達華州（State of Nevada

）的有管轄權的州法院或聯邦法院進行審理。承諾人在此不可撤銷地服從上述法院的個人管轄權（Personal Jurisdiction），並放棄以“不方便法院”（Forum Non Conveniens）為由提出異議的權利。

The Parties agree that any civil litigation or legal proceedings arising out of or relating to this Agreement shall be brought exclusively in the state or federal courts located in the State of Nevada. The User hereby irrevocably submits to the Personal Jurisdiction of such courts and waives any objection based on the doctrine of "Forum Non Conveniens."

即使本協議的任何其他條款（如免責條款、身體改造條款）被任何法院裁定為無效、非法或不可執行，本“爭議解決與管轄權”條款應被視為獨立的協議（Separable Agreement），繼續保持完全的法律效力。

Even if any other provision of this Agreement (e.g., Liability Waiver, Body Modification clauses) is held to be invalid, illegal, or unenforceable by any court, this "Dispute Resolution and Jurisdiction" Article shall be deemed a Separable Agreement and shall remain in full force and effect.承諾人簽字 (User Signature):日期 (Date):見證人簽字 (Witness Signature):日期 (Date):附錄1：

第一部

分：紋身樣式：紋身1位置在陰莖及陰莖上方兩英寸的範圍，圖案為一個特製紋身樣式。紋身2位置在大腿外側，內容如下：廢物鎖奴、喪失射精權、貞操鎖已鎖死，請檢查。

第二部

分：PA環樣式：環從尿道口（Urethra）進入，從龜頭下方的繫帶（Frenulum）處穿出。具有不可逆性和一定的健康風險。

第三部

分：鎖定裝置。鎖定裝置通過一個環固定在陰莖及陰囊根部，一個全包裹的不鏽鋼316L金屬鎖包裹並壓制整個陰莖，與PA環連接，只露出尿口。鎖芯裝置採用永久鎖芯，採用不鏽鋼316L材質，輔助器只用於一次性鎖定，無開鎖使用的鑰匙孔。這款

鎖芯提供一把不鏽鋼鑰匙，鑰匙孔有效長度 2cm，鑰匙僅作為上鎖時使用。鑰匙順時針旋轉45°方可上鎖成功，上鎖後鑰匙無效。
